Harris County Clerk’s Office - Includes forms for vital statistics, businesses, civil court, probate court, and conflicts of interest.

Harris County District Clerk - A range of forms are available, including for family, child support, civil, Texas Supreme Court, and others.

Harris County Justice of the Peace Court Forms - Downloadable forms for small claims cases, eviction, and criminal matters heard by Justices of the Peace. Commonly-used forms include small claims petition, civil answer, appeal bond, and more. Links on the left provide information about various types of cases in JP Courts. Templates are provided for your convenience - clicking the link will download the documents. Cut and paste the appropriate template for the court in which your case will be heard. Choose from District, County, County Court at Law, or Justice Court. Styles for Divorce and SAPCR cases are included.

Texas Health and Human Services Commission - Forms and information for advance medical directives, including Directive to Physicians and Family or Surrogates Form, Medical Power of Attorney Form, and Out-of-Hospital Do Not Resuscitate Form. A Statutory Durable Power of Attorney Form is also available.
